I agree to donate $25.00 for the dog documented above to the Forgotten Pet Advocates to help offset the cost of basic vetting only, which is limited to: spay/neuter, vaccines, wormer and microchip. Once my donation has been made, the Forgotten Pet Advocates will schedule an appointment for services.
Should my dog become ill it is understood that I am responsible for costs incurred.
It is understood that the Forgotten Pet Advocates does not buy or pay for animals. This is a service to help you place your rescue dog in an adoptive home. I give the Forgotten Pet Advocates permission provide necessary medical services to the dog named above.
Once vetted, I will provide a foster home for the dog named above until the dog has been adopted. The Forgotten Pet Advocates will network my dog on social media, Petfinder and Adopt-A-Pet in hopes of finding an adopter. I understand a $150 adoption fee is requested to ensure the safety of the animal. This adoption fee will be donated to the Forgotten Pet Advocates to help offset the cost of vetting.
If the said dog, should be returned at any time, he / she will go back into your home for foster.
If you are a foster fail (and want to keep the dog) and the Forgotten Pet Advocates, covered their vetting, you would be responsible for the adoption fee. If we did not cover any vetting, the fee is waived.
I promise that the information that I am giving is accurate and that the Forgotten Pet Advocates will not be held liable or chargeable for any false information or any misrepresentations that I may have submitted on this form. I understand the Forgotten Pet Advocates cannot guarantee placement.
I also agree to provide the Forgotten Pet Advocates with any and all vet records upon request.

I agree my name and contact information will/may be released to any/all prospective adopter(s) of this dog, and that the potential adopter may contact me if he/she wishes, to gain any further information on this dog.
I am aware that I have the option to request adopters to sign an adoption contract.
Upon signing this agreement, I further attest I will not surrender this dog to an animal shelter, give away for free, or euthanize this dog.
